For those students making a "first-time" request for accommodations, please complete all of the In-Take forms. These forms may be completed online and submitted electronically. Click on the link desired, place your cursor in each of the data fields and type in your information. When you have completed your form, save it to your hard drive and then attach the document in an email to: email@example.com. Your typed name in the signature field will serve as your written signature. All documents must be signed to be accepted.
For those students making a "first-time" request for accommodations, or those who have not updated their documentation with Disability Services within the last three years, please have the appropriate disability documentation form completed by the appropriate licensed professional. These forms may be printed, completed and submitted by fax, mail or direct delivery by the student. In addition your licensed professional may complete the appropriate form online and submit electronically, by clicking on the link desired, placing the cursor in each of the data fields and typing in the appropriate information. When the form is completed, it must be saved to the hard drive and then attach the document in an email to: firstname.lastname@example.org. Your and your licensed professional's typed name in the appropriate signature field will serve as written signatures. All documents must be signed to be accepted.
